Flu To and seasonal flu: In what do they differ?

Very much it has said and debated about the dangers that the flu can transport A, sorrow that is not so harmful as in the beginning he went so far as to think. HIV and AIDS


AIDS is one of the most serious, deadly diseases in human history.

More than 20 years ago, doctors in the United States identified the first cases of AIDS in San Francisco and New York. Now there are an estimated 42 million people living with HIV or AIDS worldwide, and more than 3 million die every year from AIDS-related illnesses.

AIDS is caused by the human immunodeficiency virus (HIV). HIV destroys a type of defense cell in the body called a CD4 helper lymphocyte (pronounced: lim-fuh-site). These lymphocytes are part of the body's immune system, the defense system that fights infectious diseases. But as HIV destroys these lymphocytes, people with the virus begin to get serious infections that they normally wouldn't — that is, they become immune deficient. The name for this condition is acquired immunodeficiency syndrome (AIDS).

As the medical community learns more about how HIV works, they've been able to develop drugs to inhibit it (meaning they interfere with its growth). These drugs have been successful in slowing the progress of the disease, and people with the disease now live much longer. But there is still no cure for HIV and AIDS.

Hundreds of U.S. teens become infected with HIV each year. HIV can be transmitted from an infected person to another person through blood, semen (also known as "cum," the fluid released from the penis when a male ejaculates), vaginal fluids, and breast milk.

The virus is spread through high-risk behaviors including:

* unprotected oral, vaginal, or anal sexual intercourse ("unprotected" means not using a condom)
* sharing needles, such as needles used to inject drugs (including needles used for injecting steroids) and those used for tattooing

People who have another sexually transmitted disease, such as syphilis, genital herpes, chlamydia, gonorrhea, or bacterial vaginosis are at greater risk for getting HIV during sex with infected partners.

If a woman with HIV is pregnant, her newborn baby can catch the virus from her before birth, during the birthing process, or from breastfeeding. If doctors know an expectant mother has HIV, they can usually prevent the spread of the virus from mother to baby. All pregnant teens and women should be tested for HIV so they can begin treatment if necessary.

How Can I Avoid Drinking?

If all your friends drink and you don't want to, it can be hard to say "no, thanks." No one wants to risk feeling rejected or left out. Different strategies for turning down alcohol work for different people. Some people find it helps to say no without giving an explanation, others think offering their reasons works better ("I'm not into drinking," "I have a game tomorrow," or "my uncle died from drinking," for example).

If saying no to alcohol makes you feel uncomfortable in front of people you know, blame your parents or another adult for your refusal. Saying, "My parents are coming to pick me up soon," "I already got in major trouble for drinking once, I can't do it again," or "my coach would kill me," can make saying no a bit easier for some.

It can be hard for some people to talk to adults about these issues, but a supportive person in a position to help can refer students to a drug and alcohol counselor for evaluation and treatment.

Sometimes people live in homes where a parent or other family member drinks too much. This may make you angry, scared, and depressed. Many people can't control their drinking without help. People with drinking problems can't stop drinking until they are ready to admit they have a problem and get help. This can leave family members and loved ones feeling helpless. If you have a friend whose drinking concerns you, make sure he or she stays safe. Don't let your friend drink and drive, for example. Don't get in a car with someone who's been drinking, even if that person is your ride home. Ask a sober adult to drive you instead or call a cab.

The Difficulty in Kicking the Habit

Nicotine is both a stimulant and a depressant. That means it increases the heart rate at first and makes people feel more alert (like caffeine, another stimulant). Then it causes depression and fatigue. The depression and fatigue — and the drug withdrawal from nicotine — make people crave another cigarette to perk up again. According to many experts, the nicotine in tobacco is as addictive as cocaine or heroin.

But don't be discouraged; millions of Americans have permanently quit smoking. These strategies can help you quit, too:

Put it in writing. People who want to make a change often are more successful when they put it in writing. So write down all the reasons why you want to quit smoking, such as the money you will save or the stamina you'll gain for playing sports. Keep that list where you can see it, and add to it as you think of new reasons.

Get support. People whose friends and family help them quit are much more likely to succeed. If you don't want to tell your parents or family that you smoke, make sure your friends know, and consider confiding in a counselor or other adult you trust. And if you're having a hard time finding people to support you (if, say, all your friends smoke and none of them is interested in quitting), you might consider joining a support group, either in person or online.

What's Up With Body Hair?


Body hair really gets going during puberty. Some boys will start to notice hair growing on their face around the chin, on the cheeks, and above the lip. Also, hair grows on the chest, the armpits, and even down there in the pubic region. Remember that there's nothing to worry about because hair is just one of the body's many ways of telling you that you are on your way to manhood.

You're growing hair in new places because hormones are telling your body that it is ready to change. Some of the hormones that trigger this new hair growth come from your adrenal glands. Other hormones come from your pituitary (say: puh-too-uh-ter-ee) gland (a pea-shaped gland located at the bottom of your brain). These pituitary hormones travel through your bloodstream and make your testicles ("balls") grow bigger and start to release another hormone called testosterone that also helps make your body start sprouting hair in your pubic area, under your arms, and on your face.

Boys don't really need to do anything about this new hair that's growing. Later, when you're a teen, and the hair gets thick enough on your face, you may want to talk with your parents about shaving.

When Will I Get Muscles?


During puberty, some boys might become worried about their bodies after seeing what some of their friends look like. For instance, lots of boys are concerned about their muscles. You may have already noticed some boys starting to get chest muscles (called the pectoralis muscles or pecs for short). Others may have broad shoulders (the deltoids, or delts for short). Other boys might still be slimmer and smaller.

Remember that puberty happens on its own schedule, so there's no rushing it if you're a little slower to develop muscles. Maybe you've considered lifting weights to help yourself get bigger. It's important to know that if you haven't quite reached puberty, this will tone your muscles, but it won't build up any muscles yet. Eating nutritious food and being active (like riding your bike, swimming, and playing sports) will help you be a kid who's strong and fit. In time, you'll reach puberty and you can start building your muscles, too.

If you decide to try lifting weights, first let your doctor know you are interested. He or she may tell you to hold off on weightlifting for a bit or give you some advice on how to start. If your doctor discourages weightlifting, try some other ways to work out. Resistance bands, which are like big rubber bands, are a great way to help build your strength without putting too much strain on your muscles.

If your doctor recommends weightlifting, here are some tips:

  • Have a qualified coach or trainer supervise you. It's smart to have somebody show you the proper way to lift weights. This will help you gain strength and prevent injury.
  • Use lighter weights. Your coach or trainer can recommend the right amount. Lifting heavy weights can cause injuries and then you'll have to wait until you recover before you can work out again.
  • Do repetitions. It's better to lift a smaller amount of weight a bunch of times than to try to lift a heavy weight once or twice.
  • Rest. Let your body have a break at least every other day.

Why Are Girls Taller Than Me?


You might have noticed that some of the girls you know are taller than the boys. But you've probably noticed that out of the adults you know, most of the men are taller than the women. What's going on?

Well, girls get a head start on puberty — and growing taller — because they usually start these changes between the ages of 8 and 13. Most boys, on the other hand, don't begin until between the ages of 9 and 14. So that's why girls are often taller than boys during that time.

Most boys may catch up — and even grow taller than girls. But it's also important to remember that your genetics play a role in height. So if your mom and dad are tall, you're more likely to be tall. And if your mom and dad are kind of short, you may be short, too. But nothing is definite.

You have to wait and see how it turns out, but you can also talk to a doctor if you're concerned. Remember — not every adult male is tall. Many men who are considered "short" have gone on to have careers in the movies, the military, and even professional basketball!

There aren't any exercises or magic pills to make you grow tall. But by being active and eating nutritious foods, you're helping your body grow up healthy, just the way it should.

Burning Heart Tissue Beats Atrial Fibrillation


When drug therapy can’t control the dangerous heart rhythm disorder called atrial fibrillation, burning out the cardiac tissue responsible for the abnormality will do the job in most cases, a new study has found.

Called catheter ablation, the burning technique ended episodes of abnormal heartbeat and improved the quality of life over a nine-month period for two-thirds of the people in the study who had the procedure, compared with 16 percent of those who were started on a new drug regimen.

The finding was reported in the Jan. 27 issue of the Journal of the American Medical Association.

In atrial fibrillation, the upper chambers of the heart have episodes in which they quiver rather than beat to circulate blood. The episodes are often uncomfortable, causing chest tightness, pain and other symptoms. In addition, clots can form in the stagnant blood, traveling to block arteries and causing heart attacks or strokes. An estimated 2 million Americans have the condition, which becomes more common with aging.

A variety of drugs are used to treat atrial fibrillation, but initial drug therapy will not satisfy about half the people with the condition, said study author Dr. David J. Wilber, who is director of the Cardiovascular Institute at Loyola University Medical Center in Illinois.

“What has become clear is that once you fail one drug, your chances of responding to another drug are limited,” he said. At that point, Wilber said, he might suggest catheter ablation, in which an electrical impulse is transmitted to eliminate the source of the irregular heartbeat.

The study, done at 19 hospitals in the United States and abroad, included 167 people whose atrial fibrillation was not controlled by one or more drugs. Of that group, 106 people had catheter ablation treatment and 61 were given a different drug. The trial was cut short because of the obvious advantages of catheter ablation, the researchers reported.

But that does not mean that catheter ablation is the first choice for treatment of atrial fibrillation, Wilber noted. He said he suggests it only when drug therapy clearly is not working, with the choice largely left up to the person with the condition.

“Some patients may say they want to try anything else first, some say they want the invasive procedure [catheter ablation],” he said. “This study offers the possibility of catheter ablation replacing drug therapy in these cases because the trial shows those patients do better.”

Catheter ablation does have its risks, Wilber acknowledged. Previous studies have found a complication rate of 1 percent to 5 percent, he said. No such complications were seen in the new study.

“The amount of heart tissue that is destroyed has a relatively small effect on heart function,” Wilber said. “That tissue isn’t missed by the heart as much as you might expect.”

And though many large medical centers are equipped for catheter ablation, expertise in the technique can differ, he said. “There is a very large range of experience,” Wilber said. People considering the procedure should check published reports of clinical trials for evidence of a given medical center’s expertise.

“I don’t think there is a simple way right now” to do this, Wilber said, adding that “there is considerable interest in a national registry, as for heart surgery.”

Dr. Frank Marchlinski, director of the electrophysiology program at the University of Pennsylvania and a member of the research team, said the study “demonstrates convincingly that, with catheter ablation, the outcome is dramatically better.”

Like Wilber, Marchlinski said he, too, would suggest the procedure when drug therapy isn’t working.

“I give patients that information, describe the opportunity of catheter ablation and the risk of catheter ablation,” Marchlinski said. “I tell them that there is only a 20 percent chance of long-term success with other drugs.”

But because atrial fibrillation generally is not immediately life-threatening, “it doesn’t mean with absolute certainty it has to be done,” he said. “This is an elective procedure. It is appropriate to consider it for those who do not want to take drugs.”

Intensive Insulin Therapy Won’t Boost Septic Shock Survival

Among adult septic shock patients, intensive insulin therapy to lower high blood glucose levels caused by corticosteroid therapy doesn’t reduce the risk of death, a new study has found.

The death rate for septic shock — a major complication of bacterial infections — is 60 percent within a short period, according to background information in the study. Corticosteroids are used to treat septic shock but their use is associated with elevated blood glucose levels (hyperglycemia).

In the new study, published in the Jan. 27 issue of the Journal of the American Medical Association, researchers in France examined whether intensive insulin therapy would improve outcomes among adult septic shock patients treated with hydrocortisone.

The study included 509 patients randomly selected to one of four groups: continuous intravenous infusion with hydrocortisone alone; continuous intravenous infusion with hydrocortisone plus another corticosteroid called fludrocortisone; conventional insulin therapy with hydrocortisone alone; or conventional insulin therapy with intravenous hydrocortisone plus fludrocortisone. Hydrocortisone was administered every six hours, and fludrocortisone was administered once daily for one week.

Overall, 117 of the 255 patients (45.9 percent) treated with intensive insulin therapy died, along with 109 of 254 (42.9 percent) of patients who received conventional insulin therapy. There were no major differences between the two groups in any of the secondary outcome measures, such as days spent in the intensive care unit or length of hospital stay.

The researchers also found that patients in the intensive insulin therapy group had significantly more episodes of hypoglycemia (low blood sugar) than those in the conventional insulin therapy group.

The use of a second corticosteroid offered no benefit. The death rate for patients who received fludrocortisone was 42.9 percent, compared with 45.8 percent for those in the conventional insulin therapy group.

“The current study showed no evidence to support a strategy of intensive insulin therapy aimed at maintaining blood glucose levels in the range of 80 to 110 milligrams per deciliter for treating septic shock with corticosteroids,” the researchers wrote. “The current data do not support the routine use of oral fludrocortisone in addition to hydrocortisone when physicians decide to introduce corticosteroids in the management of a patient with septic shock.”

Morphine Sulfate Solution Approved


Morphine Sulfate Oral Solution has been approved by the U.S. Food and Drug Administration to treat acute and chronic pain. Sanctioned doses are 100 milligrams per 5 mL and 20 milligrams per 1 mL, the agency said in a news release.

Although the drug has been prescribed for this use “for many years,” the agency said, this form and concentration hadn’t been approved until now.

The painkiller was sanctioned as part of the agency’s unapproved drug initiative to “make sure marketed drugs meet current FDA standards,” the news release said.

The drug is manufactured by Ohio-based Roxane Laboratories, a unit of Boehringer Ingelheim.

Victoza Sanctioned for Type 2 Diabetes



Victoza (liraglutide) has been approved to treat type 2 diabetes in some adults, the U.S. Food and Drug Administration said in a news release.

But the agency warned that the once-daily injection shouldn’t be used as an initial (first-line) treatment until additional studies are completed, since the drug may cause thyroid tumors or a rare disease called medullary thyroid cancer. People at risk for this type of cancer shouldn’t use the drug, the FDA stressed.

Victoza, among a class of medicines called glucagon-like peptide-1 (GLP-1) receptor agonists, is meant to be used along with diet and exercise to control blood sugar by helping the pancreas make more insulin after a person eats.

In clinical studies involving more than 3,900 patients, people who took Victoza had more cases of pancreatic inflammation (pancreatitis) than people who took other diabetes drugs, the FDA said. The drug should be stopped if severe abdominal pain develops or tests confirm pancreatitis, the agency said.

Victoza is produced by Denmark-based Novo Nordisk.

Keamanan informasi dalam pelayanan kesehatan merupakan ancaman baru atas privasi dan kerahasiaan informasi kesehatan pasien yang berhubungan dengan otentikasi pengguna, keamanan fisik, data kepemilikan, kontrol akses, keamanan jaringan, pendidikan pengguna dan berbagai hukum dan isu-isu etis.

Hal-hal ini dijelaskan di bawah ini:

* Dikonfirmasi user-identifikasi dan otentikasi pengguna masuk ke sistem adalah sangat penting. Peran dan hak-hak pengguna harus didefinisikan sebagai bagian integral dari keamanan sistem.
* Fisik keamanan pusat data situs: keamanan pusat data sensitif dan lokasi terminal merupakan prioritas.
